Policies, Procedures, and Required Forms

Each button (Policies, Waiver, and Release) will bring you to a required document, each of which must be signed before a student is admitted into the class.  We will have copies of these forms available for parents/guardian at the required parent/guardian meeting night.  Please read each of these documents as both students and parents must agree to these policies in order to enroll in EVDA courses.
